Authorizing changes in information on family members
Select the required hyperlinked employee code in the "Select Employee" page to authorize the changes in the information on family members.
The “Authorize Changes in Information on Family Members” page appears.
The system displays the following from the previous page.
Date Format |
The date format that is applicable to the login user. |
Employee |
The name and code of the subordinate/employee selected in the previous page. |
Request Status |
The request status is displayed as any one of the following:

Employee Remarks |
The remarks entered by the employee. |

Family Member Code |
The code of the family member |
Record Status |
The record status is displayed as any one of the following:

First Name |
The first name of the family member. |
Middle Name |
The middle name of the family member |
Last Name |
The last name of the family member |
Relationship |
The relation between the family member and the employee. |
Gender |
The gender of the family member, whether male or female. From the drop-down list box, select the required option, either “Male” or “Female”. |
Date of birth |
The date of birth of the family member. |
Dependent |
Specify whether the family member is a dependent of the employee or otherwise. From the drop-down list box, select “Yes” or “No” . “Yes”, indicates that the family member is a dependent and “No” indicates otherwise. |
Occupation |
The occupation of the family member. The system will list the options in the drop-down list box such as, Employed, Housewife, Student and Retired from Service. Select the required option. |
Marital Status |
The marital status of the family member. From the drop-down list box, select the required value . |
Military Status |
The military status of the family member. From the drop-down list box, select the required value. |
Disability |
The name of the disability, for example, eye injury, knee dislocation, head injury etc. From the drop-down list box, select the required value. The system lists in the drop-down list box, the user defined disability descriptions from the quick codes component. |
Employee Code |
The unique employee code of the family member, if the family member is also an employee of the organization. |
Citizen Identification Number |
The citizenship identification number of the family member. |
CPF Number |
The Central Provident Fund Number of the family member. |
Is Relationship Current |
Indicates if the employee’s relationship with the family member is current or otherwise. Note that the relationship will not be current in case of the death of the family member of owing to divorce if he/she is the employee spouse etc. “Yes” indicates the family member’s relationship with the employee is current, “No” indicates otherwise. |
Relationship Start Date |
Indicate the date on which the relationship came into existence. For example, a relationship starts in the event of birth in a family, adoption, or marriage. |
Relationship End Date |
If the relationship is not current, indicate the date on which the relationship was terminated. For example, in the event of the death of the family member, specify the date on which the family member died. |
Reason |
The reason due to which the family member-employee relationship became non-current. Select the required value such as, “Death” or “Other” from the drop-down list box. “Death” indicates that the relationship became non-current owing to the family member’s death. “Other” indicates that the reason for termination of the relationship is other than death. |
If reason is others, please elaborate |
If the family member-employee relationship is no more current for any reason other than death, elaborate the reason in the field. |
Passport Start Date |
The date on which the passport of family member is effective. |
Passport Expiry Date |
The date on which the passport of family member is expired. |
Attach Documents |
The documents to be attached by the HR user. |

Effective From |
The date from which the corresponding version of the record was in effect. |

From the drop-down list box, select any one of the following modification options.

Authorizer Remarks |
The remarks of the authorizer pertaining to the changes made. |
Select the “Authorize” pushbutton to authorize the request for the changes in information on family members, OR
Select the “Reject” pushbutton to reject the authorization of the request for the changes in information on family members
To view information on family members
Select the “View current record” in the “Authorize Changes in Information on Family Members” page.
The “View Information on Family Members” page appears.
